At Connie Rosenberg Care Management, we believe that building a confident tomorrow begins with careful planning today. Our “Plan Ahead with CRCM” program is designed to give individuals and families peace of mind by preparing for the future and ensuring ongoing support. Whether you’re planning for your own future or supporting a special needs adult, our team of Aging Life Care® Managers is here to guide and advocate for you every step of the way.
A one-time enrollment fee includes:
- A comprehensive assessment and intake to help us understand your unique needs
- All your information entered in our secure HIPAA compliant system
- First check-in call (up to 15 minutes)
- On-going care management as needed, billed at an hourly rate
Ongoing support includes:
- An affordable automatic annual renewal fee to stay enrolled in the program
- Annual in-person meeting (one hour minimum, billed at regular hourly rate) to maintain/build personal connection and ensure accuracy (update) of records
- Brief check-in call (up to 15 minutes) approximately six months following
